Referrals

When making a new referral to Snow Angels please read the following criteria before completing the form:

• We support Service Users who reside within Cheshire West who must be aged 65+.
• We support users who still live in their own homes and are experiencing feelings of isolation and loneliness. Unfortunately, we cannot support those who are in a care home setting.
• Please note: If a service user requires face-to-face support, there may be a delay in us providing this as we currently have a waiting list in place.
